The program is funded by NSW Ministry of Health and is delivered in 7 Local Health Districts, including metropolitan and regional areas within NSW. The program is delivered by the HammondCare Consortium, comprising of HammondCare, Scared Heart Health Services and Calvary Healthcare Sydney.
