Palliative & Supportive Care

Palliative care is a comprehensive service for those diagnosed with a life threatening illness, when relief from symptoms or pain management may be required. Our aim is to provide physical, spiritual and emotional comfort, skilled symptom control, good communication as well as support for the family both during the final stages of illness and in the bereavement period. HammondCare has inpatient units at Neringah, Greenwich and Braeside Hospital.

HammondCare’s community palliative care team in northern Sydney is a consultative service providing symptom control, supportive care and end of life care in the home. The service aims to help people stay at home during a life-limiting illness.

The Palliative Care Home Support Program provides end of life care for patients who wish to die at home or to remain at home for as long as possible. The program is funded by NSW Ministry of Health and is delivered in 7 Local Health Districts, including metropolitan and regional areas within NSW. The program is delivered by the HammondCare Consortium, comprising of HammondCare, Scared Heart Health Services and Calvary Healthcare Sydney.

HammondCare provides palliative care education and training for both residential aged care and at home settings in palliative care. Our clinical leadership is underpinned by our Centre for Learning at Research based at Pallister House in Greenwich Hospital.

Rehabilitation

HammondCare’s rehabilitation services are individually designed to assist each patient to adjust and rebuild their life to live as independently as possible. We specifically cater for individuals who have experienced life altering events such as neurological diagnosis, cancer diagnosis and cancer treatment and significant impairment after any medical or surgical illness. HammondCare offers comprehensive inpatient, outpatient and day hospital services for patients at both Greenwich and Braeside Hospitals with access to a specialised multi-disciplinary team.

Greenwich Pain Clinic

The Pain Clinic treats people with chronic pain. Treatments are designed to reduce pain and help with managing aspects of life affected by pain such as physical actives and mood and treatment options may include medications, physiotherapy, hydrotherapy, clinical psychology and a Group Pain Program.

Specialist Mental Health for Older People

At both Greenwich and Braeside Hospitals, HammondCare offers a comprehensive service for patients with mental health problems including dementia with severe behavioral disturbances or other mental health issues. The aims of the service are to provide multidisciplinary assessment, treatment and management of patients with a view to appropriate placement back into the community.
